In order to see you medical results you need to fill up your birthdate (that you gave while visiting our lab). Password you can find on small realease note that was given to you by our worker (usually before giving blood)

Format date is in standard DD-MM-RRRR (example 5th may 1900), but is some cases (depends on your browser) it can be different - you can click on date icon, and choose instead of typing date.
